Program Information

1. Complete and submit a copy of the Student Application Packet. Application Packets are made available at the conclusion of a campus tour.
2. Submit High School Transcripts if graduated. If graduation date is beyond campus visit, please submit a temporary transcript.
3. Submit current IEP, Psychological and Behavioral Evaluations.
4. Submit (3) Letters of Recommendation, one from each of the categories below:
   1.  教育
   2.  Vocational/employment
   3.  个人

An application Screening Committee will review applications submitted and select potential candidates to interview. This process will continue until a cohort of 10 have been identified or  there are no other potential candidates identified.

In addition to completing the CHOICE application, a FAFSA application is also required. We encourage you to complete this as soon as possible.

Campus tours are made available Monday-Thursday. Please call 478-289-2191 if you would like to schedule a tour. The 程序 application is available at the conclusion of a campus and 程序 tour.

Program Interviews are conducted in person. Parents are not allowed to participate 在面试中. Interviews can be set up after a campus tour.

Generally, once an interview has been completed, applicants are notified of a decision within 3-5 business days.

Students who are accepted to the CHOICE 程序 will then complete an East Georgia State College application. The admission type for students will be "Auditor."

Please note that an application will not be considered unless all the information requested is received.

Please have three different individuals complete a Letter of Recommendation. 个人 should have current working or personal knowledge of the student applying for the 程序.

When mailing Letters of Recommendation, be sure that signatures are included and placed in a sealed envelope. Please understand that documents submitted will not be returned.

• Have a documented intellectual disability (70-75).
• Maintain adult rights.
• Express a desire to attend the 选择项目 for Inclusive Learning Program:

• Demonstrate ability to live within a dorm setting independently.
• Demonstrate a desire to obtain meaningful employment.
• Undertake activities of daily living without direct supervision (e.g.个人卫生; self-administering medication, basic first aid and safety needs, etc.)
• Navigate campus independently (cross roads independently, ask for assistance when needed, communicate needs effectively).
